• Help you make appointments with your doctor or specialist.
• Keep track of your health care services.
• Help you understand your diagnosis and treatment options.
• Help with referral process to access other ancillary services like Home Health care, durable medical equipment, medical supplies, or outpatient rehab.
• Keep track of medications that are prescribed by your doctors.
• Make certain your doctors are speaking to each other about your care.
• Help you fill out complex forms or documents.
• Follow-up with you after your health care services to review any additional needs.
• Support you with locating community resources to help you with any unmet needs.
• Advocate for you to get quality care on time, efficiently, and minimize your out of pocket costs.
• Make any arrangements for transportation to access your health services.
